The Government of the Republic of Madagascar has opted for a sectoral approach in the health sector and has developed the Plan for the Health Sector Development and Social Welfare 2007-2011 (PDSSPS) consistent with the components 'health' and 'welfare' (commitments 5 and 8) of the "Madagascar Action Plan" which provides the framework for all stakeholders. AFD provides the establishment of a financial contribution of around 14 million Euros in 2008 for the health sector in Madagascar. This contribution will finance activities defined in the priorities of PDSSPS.

The objective of this mission was twofold: (a) assess the current allocation of resources in the health system, both the state resources and additional resources (including resources from the World Bank through its projects) and (b) propose scenarios for allocating resources at the decentralized levels of the health system (levels of management and production levels) for future joint funding.

Description of actual services provided by your staff within the assignment:

The consultant conducted a study to:
  • Develop and discuss the necessary specifications by describing the operational management of a future system;
  • Define the needs and modalities for strengthening various levels, especially the regional directorates of health;
  • Propose a process to change the allocation device’s envelopes;
  • Submit an envelope that AFD could allocate for this component and estimate the necessary disbursements;
  • Assess fiduciary risks and other risks and propose measures to lessen the likelihood or impact of those risks.

  • the services which should bear a cost (based on services/activities/items as defined in the Essential Health Services Package)
  • estimates of the actual cost for these services – with focus on the analysis of the cost of services at the Raha Leo Dispensary – in comparison with the cost estimates from the Essential Health Care Package work and other studies done on costing of services in the East African context
  • At Raha Leo (and selected other PHCUs identified as having introduced direct charges), the current fee structure, the range of fees and how the clinics determine the amount any given client should pay
  • the willingness and ability of the people to contribute to those costs (with focus on the catchment shehias of Raha Leo – in comparison with a few randomly selected shehias in one additional peri-urban and two rural areas)
  • the acceptable / affordable cost-sharing amounts for the majority of the people (with focus on the catchment area see above)

The overall objective of the technical assistance (TA) for the Procurement and Supply Consultancy and BGVS Management Support Services for the Ministry of Health in Suriname is to establish internationally accepted procurement and supply functions in the health sector, as well as the necessary management arrangements that will facilitate Goods Procurement Practises at the BGVS.

Description of actual services provided:
The team provided the necessary support to the MOH Pharmaceutical Policy Coordinator with the development of procurement organization and planning as well as storage and distribution of commodities. More specifically assisted the MOH and its programs in the set-up of its institutional framework for procurement of Goods, Works and Services and assisted the BGVS Board of Directors and management in formulating and executing its policy for 2007-2008, which is currently being drafted.

The focus lay on:
  • Procurement and supply management activities with regard to essential drugs and supplies at GBVS as well as at MOH and its programs;

  • To appreciate the capacity of auto-financing of the hospitals

A global report articulates the results of the nine studies in order to identify the likeness:discrepancy of the morbidity profile and the costs of service provision between hospitals. This report comes out with recommendations for the hospital network, regarding hospitalisation, hospital management and financing of hospital services.

The consultants delivered a preliminary study report for the financial model for blood banks in Vietnam and a detailed plan of work for future activities to be undertaken within the framework of the study. They presented this information in a national workshop at the National Institute of Haematology and Blood Transfusion.

Assignment: Basic Benefit Package Costing Study

The objectives of the assignment are to (i) analyse and calculate the real cost of delivering BBP services including primary and secondary health care services; (ii) analyze and calculate, for patients and population, the cost of receiving primary and secondary health care services (formal and informal private out of pocket expenditure); (iii) make recommendations for improvement of the cost-accounting and pricing methodology used by the State Health Agency based on costing exercise analysis; (iv) to determine and analyze the evolution of the epidemiological profiles at hospital and polyclinic levels.

Description of Actual Services Provided:
  1. Review and become familiar with programme documentation, specifically focusing on health sector financing, content of BBP, current organization of the provision of health services and existing service delivery costing results.
  2. Collect and summarize with the Consultants with local experience recent work done in Armenia on the cost of BBP delivery, design the necessary structure for a sample study of costs (for health facilities and for patients) and epidemiological profiles.
  3. Define the set of costing and activity information (including the tools for data collection) that should be issued by the facilities for the costing analysis of all services included in the BBP at the (i) facility level, (ii) patient level and for epidemiological studies analysis.
  4. Instruct and lead the local consultant in applying the methodology in a sample of both public and private health facilities: 4 public hospitals in Yerevan, 2 private hospitals in Yerevan, 2 public hospitals outside Yerevan, 1 dispensary, 2 polyclinics in Yerevan, 2 polyclinics outside Yerevan, 2 rural ambulatories.
  5. Evaluate the outcomes of the costing study and epidemiological analysis, done by the local consultants, and provide with the BBP services cost projections.
  6. Provide with proposals regarding (i) the opportunities and methodologies to conduct further procedure/disease/condition specific costing exercises, including a priority list established through a consultation process with relevant stakeholders; (ii) BBP content, delivery, financing improvement for year 2007 based on the outcomes of the costing study.
  7. Assist the local consultants on issues that may arise during the period of actual costing survey and analysis (including distant assistance, e.g. E-mail).
