The Government of the Republic of Madagascar has opted for a sectoral approach in the health sector and has developed the Plan for the Health Sector Development and Social Welfare 2007-2011 (PDSSPS) consistent with the components 'health' and 'welfare' (commitments 5 and 8) of the "Madagascar Action Plan" which provides the framework for all stakeholders. AFD provides the establishment of a financial contribution of around 14 million Euros in 2008 for the health sector in Madagascar. This contribution will finance activities defined in the priorities of PDSSPS.

The objective of this mission was twofold: (a) assess the current allocation of resources in the health system, both the state resources and additional resources (including resources from the World Bank through its projects) and (b) propose scenarios for allocating resources at the decentralized levels of the health system (levels of management and production levels) for future joint funding.

Description of actual services provided by your staff within the assignment:

The consultant conducted a study to:
  • Develop and discuss the necessary specifications by describing the operational management of a future system;
  • Define the needs and modalities for strengthening various levels, especially the regional directorates of health;
  • Propose a process to change the allocation device’s envelopes;
  • Submit an envelope that AFD could allocate for this component and estimate the necessary disbursements;
  • Assess fiduciary risks and other risks and propose measures to lessen the likelihood or impact of those risks.

Description of actual services provided by your staff within the assignment:
  • An analysis of the public budgets dedicated to health: (i) by level (national, regional, district or health education level), by nature of expenses (investment, operational expenses) and by type of health education; (ii) currently used distribution criteria;
  • A description and an analysis of the current allocation of additional resources’ system financed by the World Bank in the framework of its ongoing project (PDSSP) and its prior interventions: age, distribution criteria, nature of the additional resources, level of decentralisation and of decision making on the granting and use of these additional resources, financing channels, follow-up mechanisms.
  • An analysis of the respective volumes between the strictly budgetary resources and the additional resources supplied by the donors;
  • An evaluation of the actual system, and of its impact on the activity of health education and on the quality of the services rendered;
  • Describe scenarios for updating the additional system allocations to districts and health facilities.
  • Two field studies during which the consultant met with officials of the health sector at regional level as well as some health facilities.
  • A review of available studies was conducted. The consultant identified the surveys, studies and interviews necessary to obtain all the relevant information.
