  • An analysis of the public budgets dedicated to health: (i) by level (national, regional, district or health education level), by nature of expenses (investment, operational expenses) and by type of health education; (ii) currently used distribution criteria;
  • A description and an analysis of the current allocation of additional resources’ system financed by the World Bank in the framework of its ongoing project (PDSSP) and its prior interventions: age, distribution criteria, nature of the additional resources, level of decentralisation and of decision making on the granting and use of these additional resources, financing channels, follow-up mechanisms.
  • An analysis of the respective volumes between the strictly budgetary resources and the additional resources supplied by the donors;
  • An evaluation of the actual system, and of its impact on the activity of health education and on the quality of the services rendered;
  • Describe scenarios for updating the additional system allocations to districts and health facilities.
  • Two field studies during which the consultant met with officials of the health sector at regional level as well as some health facilities.

2DC is a Contract of Disendebtment and Development signed between Cameroon and France. The outcome of the 2DC project is to contribute to the rehabilitation of the health of the Cameroonian population. The objectives of this study are twofold: reinforce the quality and accessibility of the health services in the provinces and reinforce the functioning and financing of the health system. This study takes part in the framework of the implementation of a future 2DC and defines elements of the financial implementation.

Description of Actual Services Provided:
  • Definition of the procedures of budget preparation and the execution of the financial expenditure for each programme financed by C2D;
  • Identification of the procedures used for the execution of the national budget and the HIPC credits (Heavily Indebted Poor Countries);

Description of Actual Services Provided:
This study of the health sector analyses the following issues:
  1. Based on the existing literature and exchanges with the stakeholders:
    • The actual state of the health sector and its principal problems;
    • Health policy, health sector reform, improvements in health development and the obstacles for the implementation;
    • Evaluation of the total health expenditure for the last years;
    • Organisation of the Public health system and of the health care services, SWOT analysis of the public health system, the consequences of the worsening of the political and socio-economical context;
    • A brief description of the private health sector, its role in the provision of services, the control mechanisms and the regulation of private health services.
  2. Based on a field mission:
    • Description of the public and private health sector, mentioning for each principal hospital the available services, the demand, the major constraints, the available support, the priority needs and needed interventions;
    • Description of the role and involvement of the Palestinian and foreign NGO’s in health projects;
    • Evaluation of the projects and strategies supported by the technical and financial partners;
    • Degree of cooperation between the Ministry and the partners and the ongoing initiatives regarding the harmonisation of aid;
    • Priority needs that are not covered by the partners.

The main objectives were:
  • To provide an updated assessment of the level and allocation of public financing to and within the health sector. To the extent possible, given acknowledged data constraints within the sector, this will include both RGOZ and external funding.
  • To review the extent to which existing resources – government and external – are deployed in line with stated priorities as reflected in policy and strategic plan documents;
  • To provide a preliminary analysis of the feasibility of planned sectoral activities given the available resource envelope;
  • To provide support to the implementation and monitoring of the Zanzibar Poverty Reduction Plan, and to feed into the Budget Guidelines and preparation of MTEF for all the priority sectors,
  • To provide support to the achievement of poverty reduction strategy (PRS) targets and related millennium development goals (MDGs),
  • To propose further pieces of analytic work to inform fiscal policy dialogue on Zanzibar’s revenue effort, tracking expenditure and service delivery, and coherence of the development budget with PRS priorities.
Description of actual services provided:
  • Review PER FY03 findings and actions taken by the sector in response to those findings indicating unaccomplished/pending actions and reasons as well as implications and the way forward. Identify follow-up actions planned for FY2006/07.
  • Undertake preliminary assessment of the feasibility of current plans compared with the available resource envelope, and spell out the implications of these options and recommendations (e.g. scaling back targets, improving efficiency, mobilization of additional resources etc). Make recommendations regarding future activity to cost sectoral activity.
  • Propose key performance indicators to be used to monitor progress, with reference to the ZPRP and experience elsewhere.
  • Identify key technical programme activities and costings to be included in the 2006/07 Budget and MTEF, drawing upon the available documents. Comment on the allocation of the sectoral resource envelope between the various programmes.

The specific objective is to support the Palestinian National Authority, especially the MOH and its partners involved in the provision of health services, to develop a clear overview and analysis of the health sector, and to propose a set of priorities and strategic orientations to be explored and developed according to foreseeable scenarios.

It will be essential during this review to identify all local stakeholders and all local ‘support groups’ (local and international NGOs active in health, private sector support, etc.) in order to build as much as possible local partnerships and coordinated action towards best health outcomes.

The study provides the decision-makers in the Nigerian Government and the European Commission with the best information to support choices regarding the focal sectors of the future EC development cooperation with Nigeria.
  1. The study includes sectors such as: agriculture & fisheries, infrastructure, health, education, tourism, private sector, etc. As it also includes themes as good governance, trade, poverty alleviation, gender and environment, and offer suggestions about how to take them into account.
  2. The study considers how development assistance may help Nigeria address its various challenges in the context of Nigeria’s overall financial and economic constraints, and analyse the likely role of development assistance within and besides overall public finances.
Description of Actual Services Provided:
Our expert was responsible for the health sector and contributed to the main analysis as follows:
  • an analysis and the identification of the areas for possible future European Commission support, with a view to contributing to the choice of focal sectors to be funded by the EC;
  • defining the criteria by which choices regarding the focal sectors can be made;
  • suggestions for possible choices of focal sectors, information on non-focal sectors;
  • an analysis of the uncompleted past projects closed after the imposition of the sanctions in 1995 ;
  • an analysis of the macro-economic size and proportions of Nigerian economy;
  • information on the relative impact of “good-governance” and “productive” projects;
  • an analysis of public spending;
  • an analysis of the impact of the proposed approach according to gender;
  • a review of the distribution of previous EC projects by region and comparison of this with indicators of population location and poverty distribution;

The overall objective of this consultancy is to give support to the elaboration of the five-year National Health Strategic Plan (2006-2011). This plan needs to be developed within a clearly defined strategic framework for health sector reform and in consultation with all relevant stakeholders and in close partnership with the Corporate Planning Unit of the Ministry of Health, Human Services, Family Affairs and Gender Relations. Capacity in the Corporate Planning Unit (and where possible beyond) for strategic planning and programme development and management will also be strengthened.

Description of Actual Services Provided:
The team has produced the following outputs/reports:
  • National Health Strategic Plan 2006-2011: The Plan deals with 1) the health vision and health sector organisation, 2) St. Lucia’s health profile and priorities, 3) Regional health services, 4) Hospital services, 5) Health sector financing, 6) Human resources for health and 7) Health sector information needs (M&E) of the Ministry of Health.
  • Report of the socio-economic impact assessment of the Universal Health Coverage scheme: An economic and social analysis of the UHC Financing Proposal was made and its feasibility for implementation was assessed.
  • Report of the Institutional Review: The institutional review was conducted through the preparation, and discussion of, a series of discussion papers that sought to clarify a number of the institutional issues surrounding the health sector reforms.
  • A framework for an Operational Manual: Guidelines and procedures were reviewed and an operational manual of procedures was developed for the Planning Unit.
  • Report of the HMIS review: After a review of the current health management information systems, advice was provided on how the needs for information at each level of the health system might be improved.
  • Report of the Human Resources development and Sector Training Plan: This deliverable has been based on the planning processes for the four separate areas: the Priority Health Areas, the Regional Health Teams, the Hospitals and the Central Support Services.

The budget available for 2004 is limited to 300 000 EUR.

  • to identify the extent to which the Medium Term Expenditure Framework (MTEF) influences HIV and AIDS spending; and
  • to identify how the AIDS strategy is reflected in the budgets of different ministries.
