In December 2007, BRC was awarded a service contract for the provision of management, training and technical support services to the 4th phase of the Benghazi HIV/AIDS Action Plan. The agreement between the EC and BRC will be for the period till 2009, with possibility of extension.

In accordance with EC requirements a “Logical Framework” and indicative workplan were compiled in November 2007. This work plan was subsequently discussed and further operationalised during an inception workshop with Libyan stakeholders and the team of European experts held in Benghazi from 24 to 26 January 2008.

BRC requested HERA to facilitate above mentioned planning process.
The overall objective of the HIV/AIDS Action Plan for the forthcoming period will be to “improve the technical and managerial capacities of relevant Libyan health institutions to deliver an integrated package of preventive and curative HIV/AIDS services”.

Description of actual services provided:
  • Analysis of the proposed logframe. Evaluation of the progress.
  • Institutional analysis and recommendations.
  • Back-to-office report summarizing the inception workshop and making some recommendations to BRC to further improve its performance in Libya.

The project focuses on strengthening the health system in terms of geographical accessibility and improving the delivery of health services in the wilaya of Tamanrasset across 3 areas:
  • Strengthening the fight against STI / HIV / AIDS prevention and care
  • Decreasing the levels of mortality and maternal and neonatal morbidity
  • Strengthening the management of reproductive health, particularly in the field of family planning and adolescent health and youth.
The formulation mission aims to validate the data contained in the identification document and to prepare a Technical and Financial Proposal (TFP)

Description of actual services provided by your staff within the assignment:

The formulation team has worked closely with experts from the Ministry of Health, Population and Hospital Reform and with the representation of BTC in Algiers on the following activities:
  • Validating the data of the identification mission, namely the validation of the project objectives and defining (or adjusting if necessary) the results. For this purpose, the objectively verifiable indicators, along with the sources of verification and the main assumptions which could jeopardize the implementation of the project, were specified.
  • Presentation of the logical framework, including a detailed description of activities with indicators, sources of validation and critical assumptions;
  • Produce a Technical and Financial Proposal (TFP) on the basis of the logical framework with
    • Detailed planning
      • validating the activities related to the results
      • planning of the project based on expected results and activities
      • Defining the timeline
      • budgetary planning in accordance with the provisions of BTC

Assignment: Formulation of the project to support the multisectoral national plan to combat HIV/AIDS

Description:Narrative Description of Project:

In 2001 Burkina Faso and the Belgian government initiated the identification process of a multisectoral HIV/AIDS intervention. The identification report was finalised and validated in 2003. After that, a team of 3 consultants was contracted by BTC for the formulation of the project.

The general objective of the identification report was “the negative impact of HIV/AIDS on sustainable development of Burkina Faso is reduced”, while the specific objective was “the implementation of the Multisectoral National Plan to combat HIV/AIDS has improved in a sustainable way”.

The proposed project consists of three “subprojects”, each of them with a specific objective:
  1. a subproject that has the objective to reinforce the multisectoral fight against HIV/AIDS in the region of Plateau central;
  2. a subproject that has the objective to reinforce the response to HIV/AIDS in university community of Ouagadougou; and
  3. a subproject that aims at improving the operational research in the field of HIV/AIDS in Burkina Faso.
Description of Actual Services Provided:
  • Analysis and validation of the identification report.
  • Analysis of the problems and identification of solutions in a participative workshop.
  • Study on the capacity of SP/CNLS to coordinate the whole project, the capacity of putting the right human resources at the disposal of the project (integration of Centre Muraz) and the necessary financial means.

The objective of the formulation mission to Tanzania was to produce a description of the viability of the Belgian support and to indicate the main areas of attention during the project implementation.

The team prepared a document in close collaboration with the main party involved – NACP management – and in consultation with other parties, in particular the Medical Stores Department (MSD) reflecting the main considerations, conclusions and decisions with respect to potential for successful and effective implementation of the support by Belgian aid by means of:
  • a brief, policy and organisation oriented assessment;
  • a technical, operation assessment.
The mission reviewed the proposed Specific Agreement between the Belgian and Tanzanian Governments.

The Clinton Foundation works in partnership with the Ministry of Health and Social Welfare to expand access to care and treatment services for Liberians living with HIV/AIDS, and support the strengthening of the health system. The goal of the project is to improve physical access to essential health services and to strengthen policy-making and management functions of the Ministry of Health. This particular assignment addressed the procurement needs.

Description of Actual Services Provided:
  • Production of a simplified procurement plan for the World Bank Health System Reconstruction Project: improving maternal and child survival with a budget of 8.5 million USD
  • Review of all terms of reference required to be advertised to ensure compliance with World Bank procurement regulations

The objectives of the mission are to develop the HIV/AIDS components for the Health and Education Sector Programme Support Documents for implementation at provincial level with clear links to the national level.

The two component descriptions shall focus at the provincial level and emphasize the potential synergy effect (strong links) between the health and education sectors in preventing and alleviating the HIV/AIDS situation. The HIV/AIDS component descriptions shall be developed within the two Sector Programme Support Document frameworks, where support to HIV/AIDS activities at central level will be mentioned. Furthermore, the components shall provide recommendations on plans of action for the first year, pace of implementation, need for technical assistance and need for evaluation of implementation.

Description of Actual Services Provided:
There are three outputs:
  • A concise debriefing note (max. 10 pages) summarising findings and recommendations
  • A Component Description on HIV/AIDS for the Health Sector Programme Support (HSPS) Document

The European commission has been active in the health sector in Mozambique for many years. The modality of EC support has gradually changed from project support towards sector wide support. EC support aims at strengthening health systems through systems reform and related capacity building as well as through direct financial support. In order to identify appropriate EC support under the EDF-10 to strengthen institutional capacity in the health sector, a needs assessment for capacity building and a mapping of these needs was undertaken.
The objective of this consultancy was to provide advice on the outline of the next EC health and HIV/AIDS Sector Policy Support Programme in Mozambique (2009-2011), specifically in relation to the needs for capacity development for the National AIDS Council and for the Ministry of Health. Options for financial support to CNCS and sector budget support under the PROSAUDE II financing modality were explored.

Description of actual services provided by your staff within the assignment:

The following activities were performed in order to achieve the mission objectives:
  • The team met with key officials from MOH, CNCS, to identify perceived needs for support, to make a situation analysis of ‘systems development’ within MOH and CNCS, and to map available technical assistance and needs for institutional capacity building.
  • A desk study of relevant documentation on the development of the health sector, including the development of SWAP was undertaken;
  • The team assessed the extent to which EC eligibility criteria for further support were met.
  • It was suggested to include five areas of support in the EC support plan (SPSP):
    1. Proactive participation of the delegation in the MoH led sector wide approach;
    2. Financial support through the sectoral ‘Proasaude’ on-budget common management arrangement;
    3. Development of appropriate, innovative and effective approaches of institutional capacity building, including twinning arrangements with relevant public health institutions;
    4. Technical assistance to support institutional development at all levels in the health sector.
    5. Technical support to the CNCS to support an effective national response to the HIV/AIDS epidemic.
  • The team proposed to provide this support through three modalities:
    1. Financial support to Prosaude, according to national management procedures;
    2. Delegated support to CNCS, by involving a EU Member State;
    3. Specific EC procedures for providing TA.

  • Therefore, the Project commissioned a mission that first had to review the data and recommendations of these studies and next, had to conduct socio-behavioural and biological studies amongst the seafarers of the four islands in order to establish a reference situation in terms of HIV and STD prevention, to adjust common policies, strategies and programmes on the fight against HIV and to evaluate the impact of the initiative of the IOC and her partners.

    1. Analysis of the HIV epidemiological situation on the islands of the Indian Ocean and the response of each country or in the framework of the regional partnership;
    2. In-depth analysis of the reasons justifying the still relatively low level of the epidemic.

  • To provide an independent analysis of the results achieved and the activities implemented in the project co-financed by the EC in Benin and DRC;
  • To make operational recommendations to the Union, the partner countries authorities and stakeholders at NTP and National AIDS Control Programme (NACP) levels as well as the EC, on future measures and strategic lessons with regard to IHC both as operational research and care approach for TB patients living with HIV in developing countries.

  • Briefing at the Secretariat of the Union in Paris;
  • Desk review of policies and IHC documents, interviews and site visit;
  • Workshop in Paris;
  • Final report focussing on relevance, SWOT analysis, effectiveness, efficiency, impact, sustainability, continuum of care, lessons learnt. The focus is on strategic issues for future implementation; One of the main conclusions of the report is that the cooperation created by the project between the national tuberculosis and national AIDS programmes is considered a successful model, which should be appropriated by the different actors in order to maintain the project achievements and to get away from the practice of disease-specific international health financing. The social and economic research component though seems not to have received sufficient attention during the project and hence should be further developed in a second phase.

The project INT108 strives to ‘improve the quality of life of the people living with HIV by strengthening the overall treatment of HIV/AIDS patients within the framework of the decentralisation of the health system.

This review was carried out near the end of a three year funding period, to ascertain the progress the project had made towards achieving its intended outcomes and outputs, to learn lessons and to provide recommendations for the future of the project and the funding relationship. The review covered the period from January 2006 until the present time.

In 1998, a severe HIV/AIDS epidemic broke out at Benghazi Children’s Hospital. The Libyan authorities requested the support of the EU, in particular in terms of policy advice, institutional support and training. The “EU HIP Action Plan for Benghazi (BAP)” was launched in November 2004 with the aim to assure appropriate medical care for the cohort of HIV-infected children. Since then, three successive decisions taken by the Commission have supported the implementation of the BAP.
The objective of BAP 4 is to introduce a coherent and integrated methodology for HIV/AIDS prevention and treatment starting from Benghazi, and progressively extending in to the whole country over the next few years.

Description of actual services provided by your staff within the assignment:

The consultant has assisted the Libyan counterpart in consistently implementing all activities agreed under the BAP. The intervention was based on a multidisciplinary and pro-active approach developed and used under previous phases of BAP with a special emphasis on the consolidation of the results achieved and integration and harmonisation with national and local policies and programmes.

The specific services provided by the consultant were as follows:
  • Attend the inception meeting in Libya to define a detailed schedule of activities;
  • Provide support to the activities which aim to upgrade the BCIDI laboratory up to an international quality level in order to become a national reference centre. This included definition of laboratory protocols and guidelines; establish Quality Assurance policy and control procedures; implement the procedures and provide continuous service training and long distance follow up;

The Multi-sectoral HIV / AIDS Project (MAP) (102 million U.S. dollars), is the first project for the fight against HIV / AIDS that was to be implemented by the government of the Democratic Republic of Congo (DRC) through a single point of coordination of the multi-sectoral body, the National Multi-sectoral Program to fight against HIV / AIDS (PNMLS). The MAP Project was conceived as a project to support the coordination of the multi-sectoral fight against HIV / AIDS in the DRC, by supporting the implementation of the National Strategic Plan against HIV / AIDS in all provinces, both urban and rural, and in a number of varied sectors.
The objective of the MAP project is to mitigate the negative impact of the HIV/AIDS epidemic on the stabilization, recovery, and development of the country. This will be achieved by:
  • increasing the access to STI/HIV/AIDS treatment;
  • mitigating the health and socio-economic impact of HIV/AIDS at the individual, household and community level; and
  • building strong and sustainable national capacity to respond to the HIV/AIDS epidemic.

Description of actual services provided:

The objective of this consultancy was to improve understanding of the current ARV needs and supply mechanism in DRC in order to ensure continued access to treatment beyond MAP closure. Under the direction of the MAP Task Team Leader the Consultant undertook the following tasks:
  • Conduct a document review on current situation of ARV supply in DRC, including actors involved, quantity supplied, distribution schedule, geographical distribution (preferably by health zone level).
  • Review the historical and current MAP data on:
    • forecasting and quantification of ARVs
    • quantities procured
    • quantities received
    • quantities distributed
    • quantities distributed and received at provincial, district and health zone levels
    • quantities consumed (administered to patients)
  • Review the existing MAP procurement practices and distribution mechanisms, including use of regional distribution centers, assessment of its functionality and supervision mechanisms.
  • Collect information and analyze the different distribution mechanisms applied by other actors involved (Global Fund, PEPFAR, etc.).
  • Assess the existing quality assurance mechanism and provide recommendations for improvement.
  • Conduct a situational analysis of patients currently under treatment, their geographical location and level of treatment received/required.
  • Interview governmental-, donor- and other organizations involved (or to be involved) in ARV supply and establish a reliable estimate of planned and expected 2010 and 2011 procurement and supply of ARVs and related products.
  • Analyze the existing coordination mechanism among the government and principal partners involved and provide recommendations for improvement.

Considering the significant additional financial resources being made available for AIDS programs there is an ongoing debate on how international funding for AIDS is impacting health systems in low income countries in general, and pharmaceutical procurement and supply management in particular. Evidence to support the various views on this subject is still scarce.
The objectives of a chapter on “Medical products, vaccines and technologies – Procurement and Supply Chain” are (i) to synthesize the existing knowledge and evidence of the impact of international AIDS funding on the pharmaceutical procurement and supply chain; (ii) to elaborate a conceptual framework for further reflection on the impact of AIDS funding on the procurement and supply chain that captures the nature, purpose and qualitative characteristics of its component, and (iii) to identify a set of hypotheses and related evaluation framework to be used for further research.

Description of actual services provided by your staff within the assignment:
  • A systematic literature search (peer-reviewed and grey literature, electronic databases, websites of (inter)national organizations, manually screening the references of relevant journal articles and publications, contacting experts in the field);
  • Establishment of a searchable database;
  • Outline of a conceptual framework serving as guide for the quantitative, empirical study;
  • Writing of Chapter 5 “Impacts on Medical products, vaccines and technologies – Procurement and Supply Chain” (draft, final draft and final version) encompassing Synthesis of Current Knowledge of Impact, finalized Conceptual framework, Evaluation framework and proposed analyses.
