Description:Narrative Description of Project:
The general objective of the mission is to contribute (i) to the formulation of the framework for financing of the health sector respecting the principles of harmonization and alignment and (ii) to the elaboration of the different stages of implementation. The purpose is to develop a consensus between the Ministry and its partners concerning the methods of financing and the management of the SWAp; to elaborate a roadmap for activities and studies to be implemented for the set up of the SWAp and of the communal fund; and to propose the terms of reference for complementary studies.

Description of Actual Services Provided:
  • Identification of the sectors for which a harmonised sector policy is being implemented, the methods of management and financing, the successes and problems encountered and the lessons learnt for the Ministry of Health;
  • Identification of the actual modes of financing of the health sector implemented by the principal partners and their wishes in relation to the financing of the SWAp;
  • Analysis of the capacity and internal organisation of the Ministry of Health regarding the programming, the financial management and the evaluation of the activities linked to the implementation of the sector strategy;
  • Identification of the possible modes of financing of the sector policy, the type of harmonisation desired, taking into account the Cameroonian context and the willingness, wishes, constraints and requests of the government and the stakeholders;
  • Presentation of the advantages and disadvantages of each method, the preconditions, the operational implications, the foreseeable risks and difficulties and the stages of implementation. The hypothesis of communal funds was tested, in particular regarding: (i) the private sector, (ii) support to the provinces, and (iii) support to the central level of the Ministry;
  • Proposal of general modes of financing or of the management of the communal funds;
  • Identification of the nature of the expenses that should be financed by the communal funds.

  1. Health Planning
    • Projection of the resource availability for the health sector over the medium term (2004-2006)
    • Facilitation of a process among key stakeholders to identify and agree on a set of principles for setting priorities over the medium term among different types of health services and among health service inputs
    • Reviewed the existing pattern of resource allocation of the sector and estimated the cost of implementing the agreed priorities
    • Conducted an Institutional/Organizational Analysis that identifies current areas of capacity/technical assistance support; assesses the implementation of agreed sectoral priorities; and recommended priorities for ongoing technical assistance/capacity building and how this should be met.
    • Reviewed current and previous strategies employed to increase provincial government financing for health (particularly operational funding) and recommend possible strategies in this area for consideration by the HSIP Working Group.
  2. Health Financing
    • Participated in the identification of the required transformation of the HSIP Trust Account, in view of a broad participation including both donors and Government funds, and assess the potential legal issues.
    • Reviewed the common disbursement mechanisms existing under HSIP in view of all stakeholders’ requirements and made appropriate recommendations in order to evolve toward a SWAp (restricting notably specific geographic or programmatic conditions on disbursements).
    • Explored the possibilities of expanding the use of the HSIP trust account to include churches (direct performance contracts), NGOs, hospitals and/or training schools depending on the agreed sector priorities

In December 2007, BRC was awarded a service contract for the provision of management, training and technical support services to the 4th phase of the Benghazi HIV/AIDS Action Plan. The agreement between the EC and BRC will be for the period till 2009, with possibility of extension.

In accordance with EC requirements a “Logical Framework” and indicative workplan were compiled in November 2007. This work plan was subsequently discussed and further operationalised during an inception workshop with Libyan stakeholders and the team of European experts held in Benghazi from 24 to 26 January 2008.

BRC requested HERA to facilitate above mentioned planning process.
The overall objective of the HIV/AIDS Action Plan for the forthcoming period will be to “improve the technical and managerial capacities of relevant Libyan health institutions to deliver an integrated package of preventive and curative HIV/AIDS services”.

Description of actual services provided:
  • Analysis of the proposed logframe. Evaluation of the progress.
  • Institutional analysis and recommendations.
  • Back-to-office report summarizing the inception workshop and making some recommendations to BRC to further improve its performance in Libya.

Preliminary study including an institutional and organizational analysis as well as an analysis of the current funding policies of the health sector.

Description of actual services provided by your staff within the assignment:

For the formulation of the project, one mission in two phases was organised in Burundi.
  1. The organizational and institutional analysis, including among others:
    • A description of the organization of the health system at national level and of Kirundo in particular. Particular attention was given to the institutional capacity at provincial level and the relationship between hierarchical levels;
    • A description of the health system financing at various levels of the administrative services and health care;
    • A description and analysis of all instances involved in the health sector (in Kirundo and nationally), reflecting their role in the sector and the relations between these actors;
    • A quantitative analysis of human resources working in the health sector (public institutions) and their level of training;
    • An analysis of specialized training centres for basic training of health personnel;
    • An analysis of opportunities and initiatives undertaken by stakeholders to promote the harmonization of interventions in the health sector in order to improve efficiency (including a description of the policy and modes of intervention for other donors in the sector);
  2. An analysis of the current funding policies of the health sector included:
    • An analysis of the modalities and operational problems of the financing system for the decentralized health structures, and the coordination with the central level;
    • The identification procedures for contracting;
    • Identifying areas of uncertainty and risk in the new funding policy.
The team carried out the above described tasks meticulously and presented their results at a workshop for all local and national stakeholders involved in the health sector and other interest fields. This workshop helped to validate the key proposals.

The project focuses on strengthening the health system in terms of geographical accessibility and improving the delivery of health services in the wilaya of Tamanrasset across 3 areas:

  • Strengthening the management of reproductive health, particularly in the field of family planning and adolescent health and youth.
The formulation mission aims to validate the data contained in the identification document and to prepare a Technical and Financial Proposal (TFP)

Description of actual services provided by your staff within the assignment:

The formulation team has worked closely with experts from the Ministry of Health, Population and Hospital Reform and with the representation of BTC in Algiers on the following activities:
  • Validating the data of the identification mission, namely the validation of the project objectives and defining (or adjusting if necessary) the results. For this purpose, the objectively verifiable indicators, along with the sources of verification and the main assumptions which could jeopardize the implementation of the project, were specified.
  • Presentation of the logical framework, including a detailed description of activities with indicators, sources of validation and critical assumptions;
  • Produce a Technical and Financial Proposal (TFP) on the basis of the logical framework with
    • Detailed planning
      • validating the activities related to the results
      • planning of the project based on expected results and activities
      • Defining the timeline
      • budgetary planning in accordance with the provisions of BTC

Assignment: Formulation of the project to support the multisectoral national plan to combat HIV/AIDS

In 2001 Burkina Faso and the Belgian government initiated the identification process of a multisectoral HIV/AIDS intervention. The identification report was finalised and validated in 2003. After that, a team of 3 consultants was contracted by BTC for the formulation of the project.

The general objective of the identification report was “the negative impact of HIV/AIDS on sustainable development of Burkina Faso is reduced”, while the specific objective was “the implementation of the Multisectoral National Plan to combat HIV/AIDS has improved in a sustainable way”.

The proposed project consists of three “subprojects”, each of them with a specific objective:
  1. a subproject that has the objective to reinforce the multisectoral fight against HIV/AIDS in the region of Plateau central;
  2. a subproject that has the objective to reinforce the response to HIV/AIDS in university community of Ouagadougou; and
  3. a subproject that aims at improving the operational research in the field of HIV/AIDS in Burkina Faso.

  • Analysis and validation of the identification report.
  • Analysis of the problems and identification of solutions in a participative workshop.
  • Study on the capacity of SP/CNLS to coordinate the whole project, the capacity of putting the right human resources at the disposal of the project (integration of Centre Muraz) and the necessary financial means.

The overall objective of the PADSS is to contribute to the enhancement of the access (geographical, financial and cultural) and the quality of the care, at regional (regional hospital), Cercle (Referral Health Centres) and community (Community Health Centres or CSCOM) level.
The Logical Framework mentions three specific objectives (i) Contribute significantly to the enhancement of the access to care for the populations of the 5th Region and of the treatment of the patients and their pathologies; (ii) Reduce the disparities in access to health services and improve the availability of care, and (iii) Develop initial training of health staff and the management capacities of the decentralised facilities.
The approach used was a financial and technical aid to the entire health system in the region, through a direct support to PRODESS (the Health and Social Development Programme)

Description of actual services provided by your staff within the assignment:

The general objective of the mission was to evaluate the Programme to Support the Health and Social Development of the Mopti region.

The specific objectives were to:
  • Judge on the relevance of the PADSS in the 5th Region, and on the consistency of the PADSS
    1. with the sectorial health policy (from 2005 on implemented by the PRODESS-2);
    2. with its environment (regional context, other partners).
  • Analyse the institutional organisation, as well as the organisation of the programme, its management, and the role/ performance of the technical assistance SOFRECO.
  • Analyse de state of progress of the planned activities, as well as the extent of the results (including their ‘impact’ on the health system of the 5th Region).
  • Formulate concrete and pragmatic recommendations, to obtain a maximum of results during the next phase. The adopted approach was participative, allowing the main actors and the agents in the field to expose their expectations of the programme and their perceptions regarding the obtained results:
  • Meetings in Bamako with the central services and the partners;
  • Meetings in the Mopti region with (i) the Regional Health Director (DRS) and his assistants (DRDSES), the heads of department, the Assistance Advice team (AA); (ii) the administrative authorities; (iii) the representatives of the communities; (iv) the representatives of the civil society; and (v) the public health teams;
  • A participative workshop “Open Space”, bringing together the public health teams of the 8 Cercles, the two Regional Directions and the AA team;
  • The analysis of the documentation and the data of the Health and Social Information System at regional level.

Reform of the present health insurance scheme and the development of sustainable financing strategies are key elements of the reform.

The African Ophtalmological Tropical Institute (IOTA) had been in charge of the Regional Support Programme against Blindness between 1993 and 1998. The Programme had been extended with one year till 1999 when IOTA would have been handed over to the Malian government. The mid-term evaluation of the Programme had recommended to extend the transition period with one more year till 2000 in order to: (i) evaluate the national Programmes against Blindness of the 8 beneficiary countries, (ii) identify the needs in terms of technical and epidemiological expertise and (iii) to develop a network of epidemiologists in ophthalmology in the 8 countries.

A field visit to each of the beneficiary countries in order to:
  • Assess the physical and functional availability and utilisation of the equipment provided by the Programme;

Capacity in the Corporate Planning Unit (and where possible beyond) for strategic planning and programme development and management will also be strengthened.

The team has produced the following outputs/reports:
  • National Health Strategic Plan 2006-2011: The Plan deals with 1) the health vision and health sector organisation, 2) St. Lucia’s health profile and priorities, 3) Regional health services, 4) Hospital services, 5) Health sector financing, 6) Human resources for health and 7) Health sector information needs (M&E) of the Ministry of Health.
  • Report of the socio-economic impact assessment of the Universal Health Coverage scheme: An economic and social analysis of the UHC Financing Proposal was made and its feasibility for implementation was assessed.
  • Report of the Institutional Review: The institutional review was conducted through the preparation, and discussion of, a series of discussion papers that sought to clarify a number of the institutional issues surrounding the health sector reforms.
  • A framework for an Operational Manual: Guidelines and procedures were reviewed and an operational manual of procedures was developed for the Planning Unit.
  • Report of the HMIS review: After a review of the current health management information systems, advice was provided on how the needs for information at each level of the health system might be improved.
  • Report of the Human Resources development and Sector Training Plan: This deliverable has been based on the planning processes for the four separate areas: the Priority Health Areas, the Regional Health Teams, the Hospitals and the Central Support Services.

The European commission has been active in the health sector in Mozambique for many years. The modality of EC support has gradually changed from project support towards sector wide support. EC support aims at strengthening health systems through systems reform and related capacity building as well as through direct financial support. In order to identify appropriate EC support under the EDF-10 to strengthen institutional capacity in the health sector, a needs assessment for capacity building and a mapping of these needs was undertaken.
The objective of this consultancy was to provide advice on the outline of the next EC health and HIV/AIDS Sector Policy Support Programme in Mozambique (2009-2011), specifically in relation to the needs for capacity development for the National AIDS Council and for the Ministry of Health. Options for financial support to CNCS and sector budget support under the PROSAUDE II financing modality were explored.

Description of actual services provided by your staff within the assignment:

The following activities were performed in order to achieve the mission objectives:
  • The team met with key officials from MOH, CNCS, to identify perceived needs for support, to make a situation analysis of ‘systems development’ within MOH and CNCS, and to map available technical assistance and needs for institutional capacity building.
  • A desk study of relevant documentation on the development of the health sector, including the development of SWAP was undertaken;
  • The team assessed the extent to which EC eligibility criteria for further support were met.
  • It was suggested to include five areas of support in the EC support plan (SPSP):
    1. Proactive participation of the delegation in the MoH led sector wide approach;
    2. Financial support through the sectoral ‘Proasaude’ on-budget common management arrangement;
    3. Development of appropriate, innovative and effective approaches of institutional capacity building, including twinning arrangements with relevant public health institutions;
    4. Technical assistance to support institutional development at all levels in the health sector.
    5. Technical support to the CNCS to support an effective national response to the HIV/AIDS epidemic.
  • The team proposed to provide this support through three modalities:
    1. Financial support to Prosaude, according to national management procedures;
    2. Delegated support to CNCS, by involving a EU Member State;
    3. Specific EC procedures for providing TA.

The evaluation study consisted of a combined evaluation for 1) a comprehensive, and independent final review and analysis of the performances of The Second Health Sector Rehabilitation and Development Project (HSRDP II) against the Contribution Agreement with World Bank, and 2) a comprehensive, and independent mid-term review and analysis of the performances of the Support to the Implementation of the Health Sector Investment Programme (SIHSIP) against the Financing Agreement of the Contribution Agreement with World Bank, and Service Contract with BMB Mott McDonald. The main objective of this evaluation was to provide an independent assessment on the performance of the programmes, key lessons and practical recommendations for follow-up and to convey an opinion on the relevance of the programme and the feasibility of sector support for the future.

Description of actual services provided by your staff within the assignment:

For both evaluations the team used conventional components of document review, interviews with key stakeholders, and a workshop for presenting the findings and recommendations as well as gathering feedback. Key stakeholders were identified and a provisional work plan was made. The interviews were semi-structured with questions along the lines of the specific objectives.

The specific objectives for Final Evaluation of HSRDP II were achieved as described below:
  • A comprehensive assessment of the achievements of the programme was performed;
  • The lessons learned were analysed;
  • The relevance and efficiency of the programme management and implementation was evaluated;
  • The effectiveness of planning and management support, monitoring and supervision, communication, coordination, etc were examined;
  • The stakeholders’ participation was evaluated
  • The likelihood for positive outcomes to continue after completion period was assessed.
For the Mid-term Evaluation of the SIHSIP, the specific objective were addressed as follows:
  • The extent to which the SIHSIP is contributing to development in the health sector was reviewed;
  • The consistency and links between the two components was assessed;
  • The team examined how lessons and experiences can be used by national partners;
  • The quality of the technical assistance was assessed;
  • Problems were identified and possible recommendations suggested;
  • The possibility of a no-cost extension for each component and the programme as a whole was reviewed.

A pilot model of family health services has been created in 3 Governorates. This model has been operated by the Ministry of Health, the Health Insurance Organisation (HIO) and the private sector. The Ministry of Health wishes to develop a health insurance system and to reach national coverage.

The European Commission provides support to this reform through budgetary help for the Family Health Fund, annual activity programmes and technical assistance programmes. Moreover.

An external team monitors the process of the reform and gives advise to the Ministry of Health and the EC to improve the impact of the reform strategies. HERA provided the team leader to the monitoring team. Bi-annual missions were carried out between 2001-2006.

In 1998, a severe HIV/AIDS epidemic broke out at Benghazi Children’s Hospital. The Libyan authorities requested the support of the EU, in particular in terms of policy advice, institutional support and training. The “EU HIP Action Plan for Benghazi (BAP)” was launched in November 2004 with the aim to assure appropriate medical care for the cohort of HIV-infected children. Since then, three successive decisions taken by the Commission have supported the implementation of the BAP.
The objective of BAP 4 is to introduce a coherent and integrated methodology for HIV/AIDS prevention and treatment starting from Benghazi, and progressively extending in to the whole country over the next few years.

Description of actual services provided by your staff within the assignment:

The consultant has assisted the Libyan counterpart in consistently implementing all activities agreed under the BAP. The intervention was based on a multidisciplinary and pro-active approach developed and used under previous phases of BAP with a special emphasis on the consolidation of the results achieved and integration and harmonisation with national and local policies and programmes.

The specific services provided by the consultant were as follows:
  • Attend the inception meeting in Libya to define a detailed schedule of activities;
  • Provide support to the activities which aim to upgrade the BCIDI laboratory up to an international quality level in order to become a national reference centre. This included definition of laboratory protocols and guidelines; establish Quality Assurance policy and control procedures; implement the procedures and provide continuous service training and long distance follow up;

Under the “Restructuring the Blood Transfusion Service of the Ministry of Health and Population. (MoHP)” programme, funded by the Government of Switzerland and implemented by the Swiss Red Cross, there is concern about the future economic sustainability of the operations. It was therefore decided to launch a process aimed at awareness raising among senior management on the one hand and examining options to effectively strengthen the financial sustainability of the National blood System on the other hand).

Description of Actual Services Provided:
February-March:Preparation and organisation of a workshop with the aim:
  • To familiarize the participants with recent policy developments
  • To update all parties on recent achievements and conceptual, strategic, and managerial challenges that need addressing
  • To improve transparency on the current funding structure and mechanism of the national Blood System (NBS)
  • To analyse current and expected financial situation of National Blood Transfusion Service, National Blood Regulatory Authority and Hospital Blood Banks
  • To analyse various options at policy, strategic and operational level to ensure financial sustainability of the blood transfusion system
  • To prepare a realistic Plan of Action setting the key issues to be addressed before the end of the Egyptian-Swiss project mid 2007.
June: Monitoring mission with the aim:
  • To discuss and review the recommendations of the workshop with senior management of the MoHP
  • To discuss and reach agreement with the PIU and the First Undersecretary of State Curative Care on the main priority areas for legal, institutional and organisational change of the national blood system that need addressing in short and medium term
  • To define the scope of work for interdisciplinary task groups for the preparatory and implementation work of the change process.

Audit of 5 grants to Tanzania for a total of over the 447 million USD. The purpose of the audit is to assess the adequacy of the internal control system in managing the GF grants in Tanzania.

  • review the soundness of systems, policies and procedures in safeguarding Global Fund resources;
  • assess the risks the GF grants are exposed to and adequacy of measures taken to mitigate them;
  • assess compliance with Global Fund’s grant agreements, principles, policies and standards.
Description of actual services provided by your staff within the assignment:
  • Review of the Monitoring and Evaluation (M&E) systems, for HIV, TB and Malaria with a view to provide assurance about the adequacy of the systems. The work takes into consideration the national systems as well as those that relate to the Civil Society Organizations (CSOs).
  • Site visits to projects/programs of grant recipients and implementing organizations including visits to selected health facilities, treatment and testing centres, warehouses, stores, regional depots as appropriate.
  • Report including observations and recommendations on the programs reviewed and visited, systems examined, reports/technical guidelines reviewed, quality standards in service delivery centres visited.

A global report articulates the results of the nine studies in order to identify the likeness:discrepancy of the morbidity profile and the costs of service provision between hospitals. This report comes out with recommendations for the hospital network, regarding hospitalisation, hospital management and financing of hospital services.

  • Assisted the Ministry of Health in developing a new central medical stores structure as a parastatal organisation.
  • Developed a financial plan for all proposed reforms. Consensus development at a national and international workshop where proposals for reform have been presented.
The actual and functional national central medical store in Kigali is based on the feasibility study and the technical proposals made by HERA-CDP.

The main objectives of this assignment:
  • to develop alternative licensing, accreditation and certification approaches (2 different models, including the advantages and disadvantages of applying these approaches in the Armenian context;
  • to develop in detail the selected licensing, accreditation and certification approach that will address the key issues suitable for the capabilities of the Ministry, the marz level, and the health facilities themselves and suitable implementation strategy jointly with the system stakeholders;
  • to outline the resources, training and other inputs required to successfully implement selected licensing, accreditation and certification approach in Armenia.

Description of Actual Services Provided:
  • Review the licensing, accreditation and certification approaches used in Armenia in the sphere of health education and health care through revision of all related documents and interview with systems key stakeholders.
  • Based on above review the Consultant made an evaluation of the relative strengths and weaknesses of these approaches and their appropriateness in the Armenian health care and health education systems.
  • Presentation in the workshop of the international experience in licensing and certification of health professionals and licensing and accreditation of health facilities and health education program, including the experience of countries like Armenia.
  • Assessment of the current health facilities licensing system and the licensing department capacity against the requirements under the selected licensing programs as well as medium term requirements under the strengthened system, which the consultant will recommend as part of the proposed licensing, accreditation and certification strategy.
  • Development of short and medium term strategies for Armenia.
  • Recommendations for the detailed strategy for health professional licensing and certification, including the required package of necessary documents, professionals licensing procedures/mechanisms and methods (education requirements, experience requirements, indicators, required retrainings, regularity, etc.); as well as tools samples (list of specialties, including the required minimum years of professional training; licensing standards addressing qualifications health care professionals need to operate and focusing on issues of patient safety; indicators for professional qualification assessment).
  • Advise to local experts on ways to address legal inconsistencies and bottlenecks in the current legal and regulatory framework as well as on areas that may need adaptation as the health licensing systems develop.
  • Development of a monitoring tool to evaluate the effectiveness of the improved licensing, accreditation and certification after the introduction of the system.
  • The consultant worked in close collaboration with local experts, facilitating the skill transfer to these.
